Knee pain — causes, fixes & when to see a physio

Knee pain is common — and fixable. Whether it’s a sharp twinge on hills, a dull ache after long runs, stairs that feel brutal, or a swollen knee after football, we’ll help you understand what’s going on and get you moving again.

Reviewed by Rachel Royer, HCPC-registered physiotherapist and CSP member

Runner on a country road holding her knee in pain

Quick answers

  • Can I keep training?

Often, yes — by reducing volume/intensity and avoiding pain-provoking drills while you rebuild strength and control. We’ll guide a graded return. Exercise therapy and education are the primary treatments for many knee issues, including runner’s knee.

  • How long until it improves?

Many new or flare-up knee problems settle within 6 weeks with the right self-care and progressive loading. If it’s not improving, get assessed.

  • Urgent

When should I see someone urgently?

If your knee is very painful, you can’t weight-bear or move it, it’s badly swollen/changed shape, locks or gives way, or you have fever with redness/heat around the joint — seek urgent advice.

Common symptoms and patterns

  • Pain during or after running or sport (often around or behind the kneecap)
  • Outside-of-knee pain, worse downhill or on cambered roads.
  • Morning stiffness or “first-steps” pain, easing with gentle movement.
  • Clicking without pain is usually normal; locking/giving way needs assessment.

Try this now: safe self-care

Do

  • Reduce provocative loads short-term (e.g., swap hard intervals for easy running/cycling; cut hill repeats).
  • Add 2–3 strength sessions/week focused on hips, quads and calves.
  • Use short bouts of relative rest and gentle mobility; consider topical analgesics if appropriate. For osteoarthritis flares, exercise plus simple analgesics is part of best practice.

Avoid (for now)

  • Pushing into sharp pain or repeating the exact triggers (e.g., deep plyometrics, downhill sprints).
  • Relying on passive treatments alone without an active plan — passive methods such as massage or acupuncture can help manage pain but exercises and education drive long-term outcomes in most knee pain.

If you’re unsure, book an assessment — we’ll tailor this to your goals.

Book an assessment

When to see us

  • Pain that isn’t improving after 7–10 days of sensible changes.
  • Night pain, repeated giving way, or persistent locking/catching.
  • Significant swelling after activity or an awkward twist.

I wanted to give a bit of a longer review. I saw Jo about 3 weeks ago about knee pain i'd been having. After a conversation and some diagnostics Jo was able to tell me, accurately, the inury I had and provide me loads of helpful information on treatment. I returned to see Jo today and although my knee is still painful (I have a meniscus tear which will take time to heal), lots of progress has been made and its certainly headed in the right direction. I really could not recommend Jo highly enough, she's friendly, knowledgable and has really helped me. If you're in pain or stuggling with an injuriy that won't go away – get yourself booked in you won't regret it!

The review below was prepared 7 years ago…..My views about Impact Physiotherapy haven't changed….I am still very pleased with both the Pilates classes and the physiotherapists…..almost 20 years after starting……. My husband and myself have attended Pilates classes at Impact Physiotherapy in Long Eaton for well over 10 years. We highly recommend the classes run by their physiotherapists (small groups and individual attention when it's necessary to adjust exercises because of physical injury etc). When we suffered from back and knee problems respectively, we had no hesitation in contacting Impact physiotherapists for private treatment. We were both very satisfied with the outcome: following a first meeting for medical assessment, the sessions helped us get better, offering professional treatment, plenty of advice and a programme of exercises to do at home. Although sessions are quite pricey, you don't have to wait for an appointment for very long and staff don't try to book you in for more sessions than necessary. We liked their very active approach of getting patients involved in their own recovery!

I went to Impact with ongoing shoulder and knee pain that were hindering both gym practice and sleep. After only two sessions and some very helpful exercises (complete with videos), I was ready to be signed off. The support from Julie was warm, friendly, non-judgmental and totally encouraging from start to finish. Explanations and demonstrations were clear and gave me all I needed to know to carry out the exercises at home. Julie was very effective in helping me get back on track and I would highly recommend if considering whether it is right for you.

Impact physio was given as an option from Bupa when I completely ruptured my ACL last year. I found Sally Moore on the list as she had experience with ski injuries. After seeing a consultant who was not sympathetic at all and made me feel like a useless middle aged woman who didn't need an ACL, Sally was amazing. She carried out lots of tests to see what I could and couldn't do. She gave me some exercises to do which were useful. She really helped me get my confidence back, discussing whether surgery was necessary and fortunately for me it seems it is not! I have just returned from a week of skiing with no problems with my knee! Thank you Sally!

I had really bad hip and knee pain from a cricket injury and needed a last minute appointment ahead of a game tomorrow- I had an appointment with Krishna who efficiently covered my injury history and then onto the knee and hip issue. This was giving me really bad pain even walking short distances and he assessed and located the exact area to manipulate and release the pain very quickly. I went in limping and walked out the practice normally with no limp at all! He advised what to do after the appointment and moving forwards and I have booked in for 2 weeks time for a check up and to address other muscular issues. Incredibly professional, explained and listened so well and I am looking forward to addressing other issues moving forwards! Thank you Krishna!

After struggling with knee issues for 18 months, I’ve made remarkable progress in a short time thanks to the expert guidance and tailored exercise programme provided by Julie. Her deep knowledge of my specific knee condition, combined with her consistently positive and encouraging approach, made a real difference in my recovery. The clinic itself offers a great environment, with a well-equipped consultation space and gym that allowed me to perform exercises correctly and confidently. Julie is a fantastic physiotherapist, I highly recommend her.

Excellent physio provision by Fiona at Impact. Could hardly walk and after work on back and knees can stroll for 90 mins or more. Very good advice and help with practical matters such as modifying house and office to stop injuring myself further. Would recommend without hesitation: literally transformed my daily life back towards where it should be.

On three separate occasions I have been referred by my GP to Impact for a 6 session course of physio, once for knee issues and twice for different back issues. On every occasion the courses have been very effective and have returned me to good levels of mobility, enabling me to build up my fitness and maintain gym attendance. I cannot over state how important the high quality service Impact provides has had on my overall sense of well-being and fitness, enabling me to lose weight and move away from being obese to a healthy weight.

Having aggravated a previous knee problem I needed some advice before I embarked on a planned walking holiday that could potentially see me make the problem worse. Danni not only worked on my knee, but also answered all my questions and gave me the advice I was after. I now have the confidence to go away on holiday knowing what is going on and what measures to take to support my knee both in the short term and in the long-term. Hopefully a shot on me on my 400th Birkett will be heading Danni's way next week and if I need more work on the knee after my trip I wouldn't hesitate to come back.

I went to see Phil about a problem with my knee. Phil is incredibly knowledgeable and carried out some very thorough tests including an ultrasound scan and was able to advise me with some initial treatment exercise to carry out over 2 weeks and then return for a re-evaluation. My knee had much improved and Phil was able to provide additional rehab exercises. I returned a final time a few weeks later when my knee felt fine but Phil was able to additionally help by providing more information and further exercises to help strengthen the general area and prevent future injury. Very pleased with the service. Really nice friendly guy too! Thoroughly recommended. I also saw Paul at the Wilford branch years before this for a different problem and he was also brilliant.

I highly recommend the Strength & Conditioning classes at Impact Long Eaton. I have suffered with Menopause aches and pains for a few years now and was beginning to struggle with moving around without moans and groans as a young 54 year old! I thought exercise was now a no go! Another menopause gift! Combined with some weight loss the Strength & Conditioning classes really helped with building my strength back up and my confidence around weights and to push myself out of my comfort zone. Our instructor Maya takes onboard any issues you may have and the session can be tailored to individual needs. There is always variety between each session and support on how to correctly perform each type of movement. Studio is always clean, tidy and welcoming with all the right equipment provided. I recently saw Julie at Derby Uni for some physio treatment and she could see immediately the difference in my body from the classes. Thank you Team Impact x

How we help

  1. Step 1: Assess

Movement screen, strength and control tests, training review, and (where relevant) special tests for structures like meniscus or patellar tendon. Imaging is not routinely required for many knee pains.

  2. Step 2: Plan

A progressive loading programme (strength, balance, plyometric tolerance as appropriate), plus education on pacing, footwear/surface, and recovery. For patellar tendinopathy, progressive tendon-loading outperforms generic exercise.

  3. Step 3: Return

Criteria-based progression back to running/sport, not just time passed. You’ll leave with a clear plan and checkpoints.

Physiotherapist bending a patient's knee on the treatment couch to assess it

We also offer PhysiotherapyAcupunctureMassage as part of your plan — always alongside active rehab.

Did you know? We also run Pilates, Yoga and Strength & Conditioning classes at our Long Eaton clinic, and Tai Chi in Toton.

Knee conditions and symptoms

  • Likely causes

Load exceeds capacity at the patellofemoral joint (runner’s knee). Contributing factors can include spikes in mileage/intensity, reduced quad/hip capacity, and irritability of local tissues. Exercise therapy + education is first-line.

    What helps

    • Reduce total weekly load 20–40% and avoid hills/declines short-term; keep easy aerobic minutes via cross-training.
    • Begin hip + knee strengthening 2–3×/week; include runners’ knee drills.

    How physio helps

Identify aggravators, optimise cadence/technique if relevant, load-manage, and progress strength/plyo tolerance to meet your goals. Typical improvement occurs over weeks, not days.

    Quick questions

    Do I need a scan?
Usually no — diagnosis is clinical.
Should I foam roll my quads/ITB?
Useful for symptom relief, but it’s not a substitute for progressive loading.
Should I change my shoes or cadence?
A small cadence increase (about 5–7%) and running in comfortable shoes can reduce kneecap load; we’ll personalise this.
    Can taping help?
It can reduce pain short-term so you can keep training while you build strength and control.

    Ready to get moving?

    Book onlineCall 0115 972 1319
  • Likely causes

Irritation where the iliotibial band crosses the outer knee; often linked to load spikes and control/strength deficits around hip and knee.

    What helps

    • Short-term load reduction; gentle tissue work/foam rolling for symptom relief; begin targeted hip and knee strengthening.

    How physio helps

Graduated programme (mobility where needed, then progressive strengthening and return-to-run build). Expect noticeable change within ~6 weeks with consistent work.

    Quick questions

    Is stretching enough?
Helpful for comfort but strength + load management drive recovery.
    Should I stop running completely?
Often not — adjust distance, avoid steep downhills for now, and build strength alongside.
    Will a massage gun help?
It may ease symptoms temporarily; it doesn’t replace progressive strengthening.
    Is downhill or track worse?
Downhill and tight bends often aggravate ITB pain — we’ll tweak routes while you rehab.

    Ready to get moving?

    Book onlineCall 0115 972 1319
  • Likely causes

    Tendon overload relative to capacity.

    What helps

    • Reduce/modify jumping and deep knee-bend loads; start a progressive tendon-loading plan (isometrics → heavy slow resistance → power).

    How physio helps

Stage-appropriate loading, monitoring pain within acceptable limits, and pacing sport re-entry. Tendons improve with patience and progressive resistance — they need load to get stronger.

    Quick questions

    Scan first?
Imaging usually isn’t required; it’s guided by assessment and response to rehab.
    Is rest enough?
Rest alone rarely fixes tendons; they improve with progressive strength work.
    Can I keep squatting?
Usually yes within a comfortable range — we’ll adjust depth, tempo and load.
    Do patellar straps help?
They can reduce symptoms for some people; use as an adjunct while you follow your loading plan.

    Ready to get moving?

    Book onlineCall 0115 972 1319
  • What helps

    • Settle irritability (reduce deep flexion/heavy twisting), keep gentle range and strength within comfort.

    How physio helps

Differentiate irritable meniscus vs. other sources; restore strength and control so you can bend, squat and turn confidently. Many meniscal symptoms can improve without surgery with good rehab; significant mechanical locking/catching or traumatic tears may need orthopaedic input.

    Quick questions

    MRI?
Considered if symptoms persist or surgical options are being weighed — not routinely first-line.
How long to settle without surgery?
Many improve over 6–12 weeks with good rehab, depending on irritability and goals.
    Can I do strength training?
Yes — we’ll select knee-friendly ranges and progress gradually.
    Will a brace help?
It can give confidence short-term; the priority is restoring strength and control.

    Ready to get moving?

    Book onlineCall 0115 972 1319
  • What helps

    • Therapeutic exercise and education are core; consider topical NSAIDs. Manual therapy can be considered alongside exercise; some interventions aren’t recommended. Weight management and activity pacing are part of care.

    How physio helps

A progressive strengthening and function plan to improve confidence with walking, stairs and hobbies; flare-up strategies; clear progression markers.

    Quick questions

    Is running off-limits?
Not necessarily; we’ll guide load and surfaces to your symptoms and goals.
    Is exercise safe for OA?
Yes — strengthening and activity are core parts of care and can reduce pain and improve function.
Should I use a stick or pole?
If pain limits walking, a stick in the opposite hand can help while you build strength.
    Do supplements help?
Evidence is mixed; we’ll focus on proven strategies (exercise, pacing, weight management) and discuss options.

  • Likely causes

Patellofemoral irritation or quad capacity issues.

    What helps

    • Strengthening quads and hips, step-down control work, and temporary modification of aggravating tasks. Education + exercise are first-line.

    Quick questions

    Should I avoid stairs?
No, but use pain as your guide and hold the rail while we improve control and capacity.
    Are step-down exercises safe?
Yes — controlled step-downs are part of rehab; we’ll set height and tempo to suit you.
    Will a kneecap brace help?
It may ease symptoms on busy days; keep building quad and hip strength for long-term change.

  • Likely causes

Post-load irritability of joint or surrounding tissues. A badly swollen knee after injury, or swelling with fever/redness/heat, needs urgent advice.

    What helps

    • Short-term load reduction, compression, and a review of training spikes; then progressive strengthening to improve tissue tolerance. If swelling persists or function is limited, book an assessment.

    Quick questions

    Ice or compression?
Both can help symptom relief; compression sleeves are useful after runs while you dial in training load.
    When is swelling worrying?
After a twist with marked swelling, or swelling with heat/redness/fever — seek urgent advice.
Can I train with mild swelling?
Light, pain-free activity is often okay; reduce intensity and volume until swelling settles.

Knee pain: your questions answered

Book an appointment
  • Not usually. Many knee problems are diagnosed clinically and respond well to rehab without imaging. Scans are considered if symptoms persist, red flags are present, or surgery is being considered.
  • Often no — we’ll adjust volume, intensity and terrain while you build capacity. Many runners improve with graded loading and strength work.
  • They can help symptoms short-term, but they’re adjuncts. The foundation is progressive exercise and education.
  • New or flare-up problems commonly settle over ~6 weeks; tendons often need a longer, steady build. If your knee isn’t improving, get assessed.
  • Sometimes footwear changes or temporary inserts can help comfort, but they’re rarely a standalone fix. We’ll assess your needs and focus on strengthening and load management first.
  • Ice can help calm a flare or swelling; heat can ease stiffness. Use whichever improves your symptoms, alongside your exercise plan.
  • Usually yes — they’re great low-impact options. We’ll guide intensity and duration so they support recovery rather than irritate symptoms.
  • They can be considered for specific cases and short-term relief, but they work best alongside a structured rehab plan. We’ll discuss pros and cons if relevant.
  • Most knee pain improves without surgery. Referral is considered if there’s persistent mechanical locking/giving way, failure of conservative care, or traumatic injury with instability.
  • Often normal, especially without pain or swelling. We’ll check for concerning features; otherwise, strength and control work usually improves confidence.
